MASTER SERVICES AGREEMENT

This Master Services Agreement ("Agreement") is entered into between Formulation Compounding Center ("FCC") and the registering party ("User"). By using the FormuConnect portal, User agrees to access services provided by FCC solely for legitimate business purposes. User shall maintain the confidentiality of all login credentials and shall not share access with unauthorized individuals. User agrees that all data accessed through this portal is proprietary and confidential. Any misuse, unauthorized access, or violation of this agreement may result in immediate termination of access. This agreement is governed by the laws of the State of Texas.

PRIVACY POLICY & HIPAA NOTICE

Formulation Compounding Center is committed to protecting the privacy of all users and patients. All protected health information (PHI) accessed through this portal is subject to HIPAA regulations. User agrees to use PHI only as necessary for legitimate business purposes and to implement appropriate safeguards to prevent unauthorized access or disclosure. FCC maintains audit logs of all portal activity. Users are prohibited from downloading, transmitting, or storing PHI outside of approved secure channels. Violations of this policy may result in immediate access termination and reporting to appropriate authorities.

TERMS OF USE

By creating an account on FormuConnect, User agrees to use the portal solely for authorized business activities related to Formulation Compounding Center. User shall not attempt to reverse-engineer, scrape, or otherwise extract data from the portal. User is responsible for all activity conducted under their account. FCC reserves the right to suspend or terminate access at any time without notice if misuse is suspected. Accounts are non-transferable. User agrees to notify FCC immediately if they suspect unauthorized access to their account. These terms may be updated periodically and continued use constitutes acceptance of updated terms.
